Licensed Social Services Caseworker

Caseworker will:

• Identify the needs of residents
• Coordinating resources that promote self-sufficiency
• Make referrals for in-home health care and/or homemaker assistance
• Responsible for working with community partners to bring in workshops
• Maintain data on medically fragile/homebound residents
